Care Coordinator

The Care Coordinator is responsible for assisting families and individuals with their social services needs. This role requires agility and ingenuity with helping families and individuals with basic needs and connecting them to resources. The main role of the coordinator is to assist families of diverse populations with social services, run adult education classes on childcare, early child development, parenting classes and to partner with Child Protective Services in keeping families together. This role also involves coordinated health care with CNMC health care partners and generally helping clients navigate whatever crisis they may find themselves in or needs they may have.

These diverse needs include basic knowledge and understanding on services in the United States like SNAP, Medicaid and TANF. This may also include other services that have the purpose of benefiting families including emergency relief programs when necessary, teaching basic child care and feeding programs, clothing closets, etc. The coordinator is also responsible for setting up community outreach program in partnership with the Panhandle Coalition, Moore County Pregnancy center and other social service organizations in the Texas Panhandle. Specifically, it is the coordinator’s responsibility to build relationships with local services and collaborate with them on various programs and projects.
